Of course, if the doctors just reduced their fees, they could work as much as they liked without breaking any income limits. Or they could keep charging as much as ever but donate, say, one third of it to charity and then they could still work as much as they liked without breaking any income limits...

When I was in Bali a couple of years ago the street stalls were selling bamboo straws, which are reuseable so long as you can wash them and keep them clean. My impression is that most people wouldn't bother, unfortunately, so the disposable grass straws are probably a better replacement for plastic....
